1) Non-church-ministry related events require a $50 FEE BY THE DAY OF THE EVENT.
2) Filling out this form does not guarantee facility use until receiving a phone or email confirmation.
3) Weddings require additional fees, paperwork, and stipulations.

We will pay the $50 fee if applicable (for non-church-ministry related events) by the day of the event. I am responsible to return any borrowed church keys promptly after the event. We are responsible for all clean-up and resetting of tables, chairs, etc. The church will NOT be held responsible for any resulting injuries from this event. We will NOT utilize facilities, rooms, or multimedia items not specifically requested. We will NOT remove church wall decor unless approved by Laura Vachon.
